TRIVIAL PURSUITS: Origin of “All hell broke loose”

From what did the phrase “all hell broke loose” come?

The phrase was used in the epic poem, “Paradise Lost” by the 17th century English poet John Milton. The poem concerns the Christian story of the fall of Satan and his brethren and the rise of Man: the temptation of Adam and Eve by Satan and their expulsion from the Garden of Eden.

In book IV, line 918 says, “Came not all hell broke loose?”
